Case Worker needed for full-time, temporary position in Montgomery County. 

The Case Worker coordinates all inpatient and post-acute services. The Case Worker provides comprehensive case management for acutely ill psychiatric and substance abusing patients.  Case Workers collaborate with treatment teams to assess and develop treatment and discharge recommendations based on existing hospital policies and departmental procedures related to patient consent and confidentiality laws. 

This includes treatment planning, completion of assessments, collaborating with all pertinent parties to identify patients’ needs and resources/supports to assist in developing aftercare plans. Case workers will assess aftercare options and implement the discharge and referral process from day one including providing information, guidance and support in decision-making and recording all patient/family activity, interaction or intervention related to discharge planning on the medical record using the TIER system.

BSW and two (2) years of past experience ideally in discharge planning. MSW preferred. Will consider any combination of experience, education or training that would provide the level of knowledge, skill and ability required. License/certification a plus.

Knowledge of current practice and community services and skilled in negotiating with patients, families, physicians, community resources and payors to accomplish optimum outcomes for patients.

Ability and skill in heavy, detail specific and ongoing communications in both oral and written form, ability to communicate thoughts, ideas, issues and solutions positively and ability to complete a high volume of documentation in a fast-paced environment. Ability to organize and prioritize as well as work within a team and independently to achieve department outcomes.
