Social Services Director needed for full-time, Temp to Hire position in Philadelphia.

The primary responsibility of the Social Services Director is to coordinate the Social Services Department in order to meet the needs of the residents in the nursing home. The social and emotional needs are identified in relation to the resident's medical condition and stay in the nursing home. A team approach to nursing home care is utilized through interfacing with other department heads. The Director is responsible for developing positive relationships with diverse community organizations, as well as initiating and implementing marketing techniques and programs. Oversees the delivery of Social Services to residents in the nursing home.

Maintains contact with family/responsible party, involving them in the resident's total care, and providing them with non-medical progress reports when required or appropriate. Reminds the family of the policy regarding resident hospitalization. This includes contacting the responsible party of private residents in order to determine if the resident's bed will be reserved during hospitalization and notifying the Business of the same.

Helps new residents in the adjustment process to the nursing home. Maintains a case file for each resident in the nursing home, making sure that they contain current information. Documents in the medical chart resident's adjustment to social, emotional and medical aspects of his/her illness. This includes, but is not limited to, developing a plan of care, short term and long-term goals, and a discharge plan. This plan of care must be reviewed in accordance with Federal and State regulations and adjusted, if necessary, to reflect changes affecting the resident.

Completes the OBRA-87 Mental Health/mental Retardation form (PA-376) on all private pay admissions and has form on chart within three days after admission, and also completes this form for residents who are applying for Medical Assistance from Private Pay status. Acts as liaison between resident and community resources when necessary.

Develop positive relationships with other Social Services Departments and community agencies through visits to the same, telephone conversations, written correspondence and invitations to visit the nursing home. Advises resident/responsible party on procedures to be followed in applying for Medical Assistance and has knowledge of the current status of the application. Interface with other nursing home disciplines to develop an interdisciplinary care approach in the delivery of nursing home services. Coordinate interdisciplinary care plan meetings.

BSW, MSW with long term care, nursing home experience required.
